Job Summary

We are seeking a highly skilled and compassionate Registered Nurse - Pediatric Home Health Specialist to provide exceptional care to our pediatric patients in the comfort of their own homes. As a Registered Nurse - Pediatric Home Health Specialist, you will be responsible for conducting home health assessments, developing and implementing care plans, and providing education and support to patients and their families. Key Responsibilities

  • Conduct comprehensive home health assessments on pediatric patients, including newborns to teenagers
  • Develop and implement individualized care plans, taking into account the unique needs and goals of each patient
  • Provide education and support to patients and their families on various treatments, medications, and medical equipment
  • Monitor patient compliance and adjust care plans as needed
  • Perform various treatments, such as NG or GTube feedings and ostomy care
  • Maintain accurate and detailed records of patient care and progress
  • Collaborate with other healthcare professionals to ensure seamless care coordination

Requirements

  • Current and unencumbered state licensure as a Registered Nurse
  • Minimum 1 year of recent clinical nursing practice, preferably in ICU or pediatrics
  • Minimum 1 year of home health experience
  • Demonstrated passion for working in pediatrics
  • Experience with NG or GTube feedings and ostomy care is preferred
