Job Summary

We are seeking an experienced LPN to work in our Acute 24 Hours Care, Detox, 32 Hours Night Shift program. As an LPN, you will be responsible for assessing the physical and psychosocial status of patients, administering medications, and providing education and support to patients and their families. You will work closely with our interdisciplinary team to provide comprehensive care to our patients.

Key Responsibilities

* Assess the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, taking and monitoring vitals of clients on the unit, medication administration, and documentation.

  • Render and supervise care of clients on the unit, including monitoring medication and consulting with prescribers.
  • Provide appropriate health care and coordinates referrals as prescribed by providers.
  • Participate in the evaluation of client's condition and progress with team members.
  • Promote education and adherence to prescribed treatment protocols.
  • Facilitate wellness and medically related psychoeducational groups to individuals.
  • Record all client information within electronic medical record system daily.
  • Assist RN on a daily basis.
  • Offer nursing consult on emergency service evaluations.
  • Work with clients to coordinate care with their PCP, psychiatrist, and other professionals, including but not limited to medication refills and follow-up medical appointments.

Essential Qualifications

* Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.

  • Previous clinical experience working with substance abuse and mental health.
  • Demonstrated strong work ethic, ability to work independently, and ability to contribute to team objectives.
  • All bilingual candidates are strongly encouraged to apply.
  • Current valid US-issued driver's license and ability to provide registered, inspected, and insured automobile for work-related purposes, including transporting clients in your own vehicle.

Preferred Qualifications

* Experience working in a detox or substance abuse treatment setting.

  • Knowledge of medical, substance abuse, and mental health nursing.
  • Bilingual or multilingual skills.
