CVS Health is a leading health solutions company dedicated to transforming health care. They are seeking a Bilingual Analyst in Case Management responsible for assessing and coordinating care for members, ensuring their overall wellness through effective case management.

Responsibilities

• Assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness
• Effectively manage a caseload that includes supportive and medically complex members
• Develop a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration
• Determine appropriate services and supports due to member's health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports

Skills

• Bi-lingual (English and Spanish)
• Must reside in zip code 33615 or 33635, or within 2-3 miles
• Willing and able to travel 50-75% of their time to meet members face to face in Hillsborough County and surrounding areas in FL
• 1+ years of experience in behavioral health or a long-term care setting
• Computer proficiency in Microsoft Word, Excel, and Outlook
• Bachelor's Degree required, in Social Service or related field (social services field - psychology, health administration)
• Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
• Effective communication skills, both verbal and written
• Managed care experience
• Case management and discharge planning experience
