Brain Health Coach (Remote)

Job Summary ViTel Health Inc. is seeking a compassionate, personable, and reliable Brain Health Coach to join our remote clinical care team. This role is designed for a certified health coach who is passionate about brain health, prevention-focused care, and long-term patient relationships. As a Brain Health Coach, you will support patients enrolled in Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. You will begin by educating and enrolling patients into these programs and then provide ongoing telephonic and remote coaching focused on brain health, nutrition, weight management, wellness, and symptom management. This is a full-time remote position What You’ll Do Patient Outreach & Enrollment

• Contact eligible patients to educate them on RPM and CCM program benefits.
• Explain program expectations, consent requirements, and participation details.
• Guide patients through enrollment and device setup.
• Coordinate with physicians, clinics, and internal care teams to ensure smooth onboarding.
• Accurately document outreach, enrollment, and patient communication in the EHR.

Brain Health Coaching & Care Management

• Build strong, trusting relationships with an assigned panel of patients.
• Provide telephonic and virtual coaching focused on brain health and cognitive wellness.
• Coach patients on nutrition, weight management, lifestyle habits, and overall wellness.
• Encourage consistent vital sign monitoring and engagement with RPM devices.
• Monitor patient health data, alerts, and notifications.
• Develop and implement individualized coaching plans using SMART goals.
• Support patients in adhering to their physician-directed plan of care.
• Identify concerns and escalate appropriately to the treating physician or clinical team.
• Maintain regular communication with physicians regarding patient progress.
• Track all patient interactions, time, and activities in compliance with CCM/RPM requirements.

Required Qualifications

• Certification and accreditation as a Health Coach or 2+ years Health Coaching experience.
• Minimum of 2 years of experience in disease management and health and wellness.
• Education, training, or experience in neurology, brain health, or cognitive wellness
• Experience working with patients with traumatic brain injury (TBI) and/or concussions.
• Excellent written and verbal communication skills.
• Strong empathy and ability to build rapport with patients.
• Ability to work independently in a fully remote environment.
• Comfortable using technology to deliver compassionate, patient-centered care.

Preferred Qualifications

• Experience with telehealth or telephonic coaching.
• Understanding of Remote Patient Monitoring (RPM) and Chronic Care Management (CCM), including documentation and billing requirements.
• Familiarity with Social Determinants of Health.
• Current or prior licensure/certification as an LPN, Medical Assistant, Nursing Assistant, Social Worker, Pharmacy Technician, or related role.
• Background or strong interest in nutrition, integrative medicine, or lifestyle medicine.
• Interest in leadership opportunities and long-term career growth.

• Full-Time Monday to Friday
