Experienced Licensed Social Worker – Remote Medical Case Management

Job Summary We're seeking an experienced licensed social worker to join our remote medical case management team. As a licensed social worker, you'll work with a registered nurse case manager to assess, plan, and advocate for members with financial, transportation, safety, and housing needs. This role requires a collaborative approach, and you'll work closely with other clinical staff to assist members with community resources for medical, financial, and behavioral health.

Responsibilities

• Research available options and resources for members with financial, transportation, safety, and housing needs
• Collaborate with registered nurse case managers to assess, plan, and advocate for members
• Empower members to make positive lifestyle and medical choices
• Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Conduct telephonic or electronic exchanges with members to provide support and guidance
• Develop and implement individualized plans to meet members' needs
• Monitor and evaluate the effectiveness of plans and make adjustments as needed Requirements
• Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
• 2+ years of experience in medical social work or case management
• Masters degree in social work or a related field Preferred Qualifications
• Experience working with diverse populations, including children, adults, and seniors
• Knowledge of community resources and services for medical, financial, and behavioral health
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
