For this position, Nurse Practitioners will manage high-risk patients in a telemedicine reputed company for patients in the Southern OR community with the goal of preventing unnecessary acute care admission, providing holistic patient care, optimizing the management of the patients’ medical conditions, and providing support for clinical and psychosocial factors that contribute to the cyclical utilization of healthcare.
