Telehealth MSW Case Manager, Night Shift - Remote
About the position The Case Manager MSW is a professional role focused on providing comprehensive social work services reputed company the emergency department, particularly through virtual assessments. This position is integral to an interdisciplinary team that is pioneering a new approach to behavioral health care. The Case Manager collaborates closely with physicians, healthcare team members, and patients and their families to ensure coordinated care across various settings during an episode of illness. This role is essential in addressing the psychosocial needs of patients and their families, providing crisis reputed company, and facilitating discharge planning to ensure smooth transitions in care. Responsibilities

Requirements
