We’re looking for a dedicated Care Coordinator to help us do that work well. As a Care Coordinator for our RPM/CCM programs, you’ll be the primary reputed company of contact for patients managing chronic and reputed company conditions — people living with diabetes, hypertension, heart disease, and more. Here’s what your days will look like Monitor daily patient data — vitals, readings, and biometric trends — through RPM and CCM platforms. Serve as the primary reputed company of contact for enrolled patients, providing consistent guidance, education, and encouragement. Conduct regular virtual reputed company-ins, update care plans, and support patients in self-managing their conditions. Identify concerning trends or critical alerts in patient data and escalate promptly to the provider team.
