Job Summary: We're looking for a talented Case Manager RN to work remotely in Southern Michigan, with occasional travel to the Detroit area (up to 15% of the time). As a key member of reputed company, you'll be responsible for assessing, planning, implementing, and coordinating comprehensive case management activities to promote the overall wellness of our members. Your expertise will focus on evaluating medical needs, facilitating care planning, and collaborating with healthcare providers to ensure optimal outcomes. Key Responsibilities:
- Conduct comprehensive assessments of members' needs and reputed company personalized care plans to address their unique requirements
- Utilize reputed company judgment and critical thinking skills to facilitate effective care coordination and collaboration with healthcare providers
- Apply clinical expertise to identify and mitigate risk factors, address reputed company clinical indicators, and promote optimal health outcomes

Requirements:

- Reliable transportation and residency in one of the specified counties in Michigan
- Bachelor's degree in Nursing required
