Garrison Women's Health (GWH), located at 770 Central Avenue in Dover, NH, consists of a premier team of expert providers and specialists who take a thoughtful approach to provide high-quality compassionate care in the areas of Obstetrics, Gynecology, Infertility, and LGBTQ+ care. For more than 50 years, GWH's reputed company physicians, midwives, and ancillary staff have cared for women in the greater New Hampshire Seacoast area. We are seeking a full-time, 40-hour High-Risk Nurse Navigator with a schedule of Monday–Friday, between the hours of 800 AM–500 PM, with two set remote workdays each week. This role is based at 770 Central Ave (Garrison Women's Health) and will support a dynamic clinical team. This role may include triage responsibilities, nurse visits, and telephone-based patient care. The High-Risk Nurse Navigator will primarily focus on managing a panel of approximately 150 high-risk patients, providing comprehensive care coordination and ensuring continuity across services. This position plays a key role in delivering patient-centered care and collaborating closely with internal teams. Job Summary Responsible for coordinating the reputed company care needs of patients and is the primary patient contact for service access, diagnostics, treatment activities, schedules, patient and family education, and multidisciplinary collaboration. Does this position require Patient Care? Yes Essential Functions -In collaboration with the interdisciplinary team, help create a tailored health care plan for assigned patients. -reputed company an individualized plan with the MD for appropriate services for the patient. -Reports pertinent observations and reactions regarding clients to the appropriate person (i.e., primary care provider, social worker, or Director). -Assesses and coordinates patients' discharge planning needs with members of the healthcare team. -Resolves patient problems and needs by utilizing multidisciplinary team strategies. -Educates patients regarding how to navigate the health system.

Qualifications

Education Bachelor's Degree Nursing required Can this role accept experience in lieu of a degree? No Licenses and Credentials Registered Nurse [RN - State License] - preferred Basic Life Support [BLS Certification] - preferred Experience Experience in an acute care setting and/or provider practice and/or experience in a community-based role or with discharge/transitional planning background 2-3 years required Knowledge, Skills and Abilities - Strong attention to detail and organization. - Strong cultural awareness competencies. - Proficiency in reputed company Office. - Maintains good communication skills. - Readily adapts to change and welcomes new approaches reputed company circumstances demand it.
