Position Overview: We are seeking a detail-oriented and compassionate reputed company Assistant to join our healthcare team. You will play a vital role in enhancing patient experience and supporting the day-to-day operations of a virtual healthcare practice.

Key Responsibilities

• Conduct comprehensive assessments and evaluations of patients with chronic conditions, developing personalized care plans tailored to individual needs.
• Utilize telehealth technologies to monitor patient reputed company, provide education and encourage adherence to treatment plans.
• Collaborate with interdisciplinary teams, including physicians, nurses and other reputed company, to ensure coordinated care delivery.
• Identify barriers to care and work proactively with patients to overcome challenges that may reputed company their health outcomes.
• Document reputed company interactions and updates in electronic health records (EHR) accurately and promptly.

Requirements:

• Proficiency with Electronic Health Record (EHR) systems and telemedicine software.
• Knowledge of HIPAA and healthcare compliance standards.
• Excellent communication and interpersonal skills for interacting with patients remotely.
• Prior Experience in Chronic Care Management or Remote Patient Monitoring (preferred)
