We are seeking a full-time, bilingual, and remote Licensed Vocational Nurse (LVN/LPN) to join our rapidly growing team as a Care Coordinator. You will be responsible for telephonically delivering high quality care, coaching, and appointment scheduling to assigned patients. Responsibilities: Remotely provide patient health coaching and care to improve patient outcomes Coach and educate patients on improving their Chronic Conditions, preventive care and physician directives Update and manage patient care plans based on assessment of patients needs and physician directives Uphold scheduling flexibility between the hours of 8:00am 7:00pm (PST) to call patients who live in different time zones Collaborate with departments across the organization to support the successful implementation of new service lines and functions Requirements: Valid Vocational Nurse (LVN/LPN) License Valid Vocational Nurse (LVN/LPN) License - in the state of California is mandatory 2 years+ Licensed Vocational Nurse (LVN/LPN) experience Bilingual (Fluent in English and Spanish) Experience working with EHRs/EMRs Strong knowledge and background of chronic conditions Ability to build rapport with patients Compassionate and caring demeanor Excellent documentation and verbal communication skills Intermediate to advanced computer skills with the ability to navigate multiple systems Reliable internet connection Apply Job!
