Remote Entry-Level Nursing Role – Care Management Opportunity

As part of our commitment to innovation and excellence, we are expanding our Chronic Care Management (CCM) program and are seeking a Registered Nurse (RN) to support our care management team remotely. Key Responsibilities: • Assist patients by providing information, coordinating services, and offering support remotely. • Conduct outreach to patients for follow-ups, wellness checks, and care coordination. • Maintain and update medical records, ensuring accuracy in electronic medical records (EMR) systems. • Support physicians and care managers by relaying patient updates, scheduling appointments, and ensuring smooth care transitions. • Educate patients on managing chronic conditions and adhering to treatment plans. • Maintain patient confidentiality and comply with HIPAA and healthcare regulations. • Stay updated with industry best practices in telehealth and chronic care management.
