Weekend Referral/Intake Specialist – Join Our Fast-Paced Home Health Team! This is a remote position. Ideally the candidate resides in the Tucson area but not required. Applicant MUST have home health intake experience! Are you a self-starter with a passion for patient care and strong attention to detail? We’re looking for a Weekend Referral/Intake Specialist to join our dynamic and growing home health agency. - Manage weekend referrals from start to finish—review, scrub, verify, and coordinate
- Ensure compliance with PDGM guidelines, insurance requirements, and intake policies
- Obtain initial service authorizations and validate coverage
- Communicate with referral sources and clinicians to set patients up for seamless weekend admissions
- Help drive our mission forward by supporting patients as they transition into home care
Schedule: 8–10 hours each weekend (flexible based on referral flow) Key Responsibilities:
- Process and manage weekend referrals in accordance with state, federal, and ACHC standards
- Ensure accurate and compliant documentation and coding
- Collaborate with referral partners and field staff to meet patient needs
- Maximize reimbursement through insurance verification and prior authorization
- Coordinate with multiple providers to initiate care plans
- Be a knowledgeable point of contact for resources within and outside our agency
What We’re Looking For:
- Minimum 6 months experience in a home health intake role
- Strong communication and coordination skills
- Ability to work independently and prioritize tasks
- Bilingual a plus
- Passion for service, efficiency, and quality care
