Care Management RN: Remote Full Time $40

GHR has a wonderful Care Management RN opportunity for a Registered Nurse working remote in the PA TRI STATE AREA!

MUST have a PA, NJ or DE license

Registered Nurse Job Details:

• Pay Rate: Starting at $40/hour
• Shifts Available: Full Time 9a-5p
• Infusion Therapy Team
• Perform telephonic or onsite review of hospital admissions
• Encourage efficient and high quality use of the healthcare delivery system
• Promote timely and dynamic discharge planning to facilitate early discharge
• Efficiently collaborate with the hospital utilization review department

Registered Nurse Job Requirements:

• Minimum 3 years of acute care clinical experience in a hospital or healthcare setting
• Prior discharge planning and/or care management experience REQUIRED
