Job title: Licensed Practical Nurse Company: Capital Health Job description : Responsibilities Provides direct patient care utilizing the nursing process under the direction and supervision of the Registered Nurse. Reviews diagnostic test results and reports abnormal results and changes in patient condition to the appropriate healthcare team members. Collaborates with the Registered Nurse on development and implementation of the patients plan of care. Administers prescribed medications, therapies and treatments in accordance with Licensed Practical Nursing Standards. Maintains and reviews patient medical records, notes, and physician orders. Documents all patient care activities. Assists physicians, Registered Nurse and multidisciplinary team with data collection, planning, intervention and evaluation of patient and family care to achieve desired outcomes. Implements components of the plan of care with the Licensed Practical Nurse scope of practice and competency to promote desired outcomes. Assists with discharge planning and carrying out the patients teaching plan under the direction of a Registered Nurse. Requirements Graduate of an accredited Licensed Practical School of Nursing Program.
