Key Responsibilities

• Coordinates and oversees reputed company aspects of patients' home care, ensuring that their needs are met and that they receive high-quality care.
• Collaborates with patient care teams to reputed company and implement care plans that are tailored to each patient's unique needs.
• Educates families on patient condition, needs, and care, empowering them to take an active role in their loved one's healthcare.
• Performs head-to-toe physical assessments, checks vital signs, and manages medications to ensure that patients receive comprehensive care.
• Gives injections, IV therapy, or tube feedings as needed, and administers and maintains catheters to support patients' medical needs.
• Completes documentation timely and accurately, ensuring that patient records are up-to-date and reflect the care provided.
• Supervises LPNs and HHAs, providing guidance and support to ensure that patients receive seamless care.

Qualifications and Requirements To be considered for this role, you must meet the following qualifications:

• One year of clinical experience as a registered nurse (RN). If you have less than 1 year of clinical experience, you must be approved by our VP of Clinical Services.

• Minimum six months of home health experience, demonstrating your ability to provide care in a home setting.

• Familiarity with general use and functions of the computer, including user names and password concepts, internet, e-mail, and navigation of computer desktop.
